Phentermine vs. GLP-1 Injections: What’s the Real Difference?

With weight loss medications becoming more mainstream, it can feel overwhelming to figure out which option might be right for you. Two of the most talked-about names in the weight loss world are Phentermine and GLP-1 receptor agonists, like Wegovy, Ozempic, or Mounjaro. While both are used to support weight loss, they’re actually very different in how they work, how long they’re used, and who they’re best for.


Phentermine: A Traditional Tool

Let’s start with Phentermine. It’s been around for decades and is one of the oldest FDA-approved medications for weight loss. It works as a stimulant, similar to an amphetamine, by targeting the central nervous system to suppress appetite. People who take phentermine usually notice reduced hunger and a bit more energy. It’s affordable, widely available, and generally best for individuals who are otherwise healthy but need help controlling cravings and portion sizes.


GLP-1 Injections: The New Era


Now let’s talk about the newer kids on the block, GLP-1 receptor agonists. These include medications like semaglutide (brand names Ozempic and Wegovy) and tirzepatide (Mounjaro/Zepbound), and they work very differently. Originally developed for type 2 diabetes, these drugs mimic a hormone called GLP-1 that your gut naturally produces. GLP-1 helps regulate blood sugar, slows digestion, and, most importantly for weight loss, reduces appetite and increases feelings of fullness. These medications are given as weekly or daily injections, and they’re intended for long-term use.


Comparing the Two


When it comes to results, GLP-1s tend to deliver more significant weight loss. Many people lose 15–20% or more of their body weight with consistent use, especially when paired with healthy lifestyle changes. Phentermine, by comparison, usually results in 5–10% total body weight loss over a few months. That’s not to say one is better than the other–it all depends on your goals, health history, and what your body responds to.

There are also differences in side effects. Phentermine can cause increased heart rate, anxiety, insomnia, and dry mouth, since it’s a stimulant. GLP-1 medications are more likely to cause nausea, constipation, or other GI issues, especially at the beginning, but they aren’t stimulants and don’t usually affect your sleep or mood the same way that phentermine does.

Cost is another factor to consider. Phentermine is generally very affordable, but GLP-1s are often expensive and not always covered by insurance, especially if you’re using them solely for weight loss and not for diabetes. That said, some insurance plans are beginning to cover them more often as their weight loss benefits become widely recognized.
